Patients rights to participate in decisions about their healthcare

Dr Jacob encourages their patient’s rights to participate in decisions about their healthcare. If you have any questions regarding the purpose, importance, benefits, risk and possible out of pocket costs associated with your treatment don’t hesitate to speak with Dr Jacob. There is also a large range of brochures and leaflets available to support your decisions in the waiting room.

Phone access to care

Patients of our practice are able to obtain advice or information related to their clinical care by telephone or electronic means where our GP’s determine that this is clinically safe and that face to face consultation is unnecessary for that patient. To be eligible for a telehealth consultation, there is specific criteria that needs to be fulfilled. This will ensure best medical practice. Patients of our practice have the right to access their personal health information on request with permission of the Doctor.

Privacy

In line with the National Privacy Principles all patients’ consultations and Medical records are strictly confidential. It is the policy of this practice to maintain security of personal health information at all times, and to ensure that this information is only authorised to members of staff.

This Practice uses a recall and reminder system. Please let us know if you do not want to be part of this system. We have a communication policy in place including the receiving and returning of telephone calls. Patients are encouraged to make an appointment to see the Doctor regarding the results of their tests or specialists reports.

Patients who require translation

Patients who do not speak or read English or who are more proficient in another language, or who have special communication needs are offered the choice of using the assistance of a language service to communicate with the Doctor. Please notify the receptionist if you need this service.

After hours

When the surgery is unattended for urgent medical advice, ring Dr Jacob on 0409 049 754. If Dr Jacob is unavailable please contact Sydney Medical Service Co – Operative Limited on 8724 6300 or The Sutherland Hospital on 9540 7111.

Fees

Our Practice is a mixed billing Practice.  The following groups will continue to be bulk billed: • Children under the age of 18 • Pension/Concession Card Holders • DVA Card Holders • University Students/Unemployed.  Individuals that don’t fall under any of these categories, fees will apply. Fees are as followed: Level A (short consultation) – $50.00 (Patient Rebate $19.60) Level B (standard consultation) – $80.00 (Patient Rebate $42.85) Level C (long consultation) – $120.00 (Patient Rebate $82.90) Level D (extra long consultation) – $158 (Patient Rebate $122.15.  Medicare rebate available.

Feedbacks and Complaints

Any complaints and suggestions can be conveyed to Dr Jacob through the receptionist’s. We have an anti – discrimination policy in place.

Reminder System

We phone patients to remind them of their appointments and any tests/immunisations that are due. Our practice is committed to preventative care. We may issue you with a National/ State or Territory reminder notice from time to time offering you preventive health services appropriate to your care. If you do not wish to be part of this system please let us know at reception.

Appointments

We run an appointment system and endeavour to keep as close to this as possible. Patients with an appointment will have priority over those who do not, however urgent medical problems will always be dealt with promptly. If you need a longer appointment, please advise the receptionist when making your appointment.

Results

The doctors will advise when your results are expected to arrive at the practice. Please phone the practice to find out your results and ask what they mean for your care. Doctors prefer that all patients return to the practice for their results and ongoing care.
